Compliance Requirements

All undergraduate, master's, postgraduate certificate, Ph.D. and DNP students are required to submit SDSU College of Nursing compliance documents. Documents must be submitted by the stated deadlines for each program. If documents are missing, students will not be allowed to participate in clinical experiences and each absence will be considered unexcused.

To better serve our students, the SDSU College of Nursing partners with CastleBranchto manage our students' health compliance documents. The management systems allows students to meet the requirements of the agencies where they fulfill the clinical requirements of their coursework with ease by uploading required documents from their own personal computer or device.

Once admitted to your Nursing program, you will be given a package code and instructions to order your CastleBranch profile through CastleBranch.

Do not send compliance documentation to the College of Nursing. All compliance records must be uploaded into your online CastleBranch profile. Records can also be faxed or sent via mail to your CastleBranch profile.

Undergraduate Requirements

Due prior to enrollment. Some documentation will be required to be renewed on an annual basis.

SDSU College of Nursing does not guarantee clinical placement for students who are not in compliance with clinical facility requirements.

Completion or progression in the program may be delayed or impeded for students who are not in compliance. Clinical facilities may change their compliance requirements at any point in time. Students are expected to be in compliance with any changes.

SDSU College of Nursing certificate of health: The student is required to obtain the signature of a licensed health care professional stating that the student is physically able to participate in the nursing program. The certificate of health must be completed using the SDSU College of Nursing certificate of health form and must be signed by a health care professional.

Criminal background check: The student must order a criminal background check through CastleBranch. An annual background check is required.

FBI background check: The student will submit fingerprint to CastleBranch to complete the FBI background check. Fingerprint cards are available through CastleBranch or the Department of Nursing Student Services. Local law enforcement agencies may charge a fee to complete the fingerprints. Results may take 10-15 weeks.

10-panel drug screen: The student will order their drug screen through their CastleBranch account. Drug screen must be completed at a laboratory that is contracted with CastleBranch.

Cardiopulmonary Resuscitation certification (CPR): Certification must be through an American Heart Association healthcare provider or an American Red Cross professional rescuers course. Certifications issues in accordance with AHA curriculum are acceptable. The CPR card must be signed. Certificates or letters from provider are accepted temporarily (90 days). The renewal date will be set according to the expiration date of the student’s card.

Tuberculin (TB skin test): Complete documentation for the TB skin test includes the date administered, date read and negative results OR negative Quantiferon blood test and test date OR results of clear chest x-ray. The student’s initial TB skin test must be 2-step; 1-step required for annual renewal. If it has been more than 12 months since the student’s previous TB skin test, a new 2-step will be required. Annual renewal date is based on previous test date.

Immunizations:

  • Hepatitis B: The Hepatitis B vaccine is a series of 3 doses. Complete documentation includes the date of the first, second, and third doses of the vaccine OR a positive Hepatitis titer (lab report required).
  • MMR (Measles, Mumps, and Rubella): Complete documentation includes the date of the first and second doses of the vaccine after OR positive MMR titers.
  • Varicella (Chicken Pox): The Chicken Pox vaccine requires 2 doses. Complete documentation includes the date of two doses of the vaccine OR a positive Varicella titer (lab report required).
  • Primary DTP Series: Complete documentation includes at least 4 vaccinations.
  • TDaP (Tetanus, Diphtheria, and Pertussis): Documentation of a TDaP booster within the past 10 years. Renewal date set for 10 years from the administered date of the booster.
  • Polio: Documentation of the completed primary series (at least 3 vaccines) OR positive antibody titer.
  • Influenza: Documentation of a flu shot administered during the current flu season OR submission of the SDSU exemption request based on documented medical or religious reasons. Exemption requests must be submitted annually.  If exemption is approved, additional requirements such as testing and masking may be in place and clinical site placement may be limited.  If placement cannot be made for clinical based on the vaccination requirements of our clinical partners, completion or progression in the program may be delayed or impeded. The renewal date is November 1 of every year.
  • COVID: Documentation of completed COVID vaccination series (2 shots for Pfizer/Moderna, 1 shot for Johnson and Johnson) OR submit the SDSU exemption request based on documented medical or religious reasons. Exemption requests must be submitted annually.  If exemption is approved, additional requirements such as testing and masking may be in place and clinical site placement may be limited.

Health Insurance: Complete documentation includes a health insurance card with student name and policy number OR proof of coverage. If name on card does not match the student’s name, verification of coverage from the provider is required. The renewal date will be set for the expiration date on the insurance card. If there is no expiration date, the renewal date will be set for 1 year from the date of submission.

Multi-purpose release form: A signed copy of the multi-purpose release form is required.  The multi-purpose release form address the following policies from the nursing student handbook:

  • Photo release
  • Authorization to release information
  • Health insurance coverage
  • Medical expenses
  • Reporting criminal offenses
  • Student handbook
  • Technical standards
  • Progression standards
  • Laboratory equipment

HIPAA training: Due August 1 for fall admission; January 1 for spring admission. Online HIPAA training is available through CastleBranch certificate of completion must be uploaded to CastleBranch.

RN License: Submit a copy of your current RN license or verification through the state website. Renewal date will be set based on the expiration of your license.

Graduate Requirements

Didactic portion can be completed online (if available) but SKILLS demonstration MUST be in-person. Certifications issues in accordance with AHA curriculum are acceptable. The CPR card must be signed. Certificates or letters from provider are accepted temporarily (90 days). The renewal date will be set according to the expiration date of the student’s card.

Tuberculin (TB test): Complete documentation for the TB skin test includes the date administered, date read and negative results.

  • Student's initial TB test must be one of the following:
    • 2-step TB skin test (two tests within 1-3 weeks) within the past 12 months OR
    • QuantiFERON gold blood test OR
    • T-spot blood test
  • If TB test is positive, the following is required:
    • Provide the positive TB test result or a physician verification is acceptable
    • Clear chest x-ray every 5 years
    • Annual physician verification stating no TB symptoms
  • Annual TB test required:
    • 1-step TB test required for annual renewal as long as it is completed prior to the due date. If it has been more than twelve months since the student's previous TB test, a new 2-step TB will be required. Annual renewal date is based on previous TB test date.
    • If it has been more than twelve months since the student's previous TB test, the following is required:
      • 2-step TB skin test (two tests within 1-3 weeks) within the past twelve months OR
      • QuantiFERON gold blood test OR
      • T-spot blood test
    • If the annual TB test is positive, the following is required:
      • Provide the positive TB test result or a physician verification is acceptable
      • Clear chest x-ray every 5 years
      • Annual physician verification stating no TB symptoms

  • MMR (Measles, Mumps, and Rubella): Complete documentation includes the date of the first and second doses of the vaccine after OR positive MMR titers.
  • Varicella (Chicken Pox): The Chicken Pox vaccine requires 2 doses. Complete documentation includes the date of two doses of the vaccine OR a positive Varicella titer (lab report required).
  • Primary DTP Series: Complete documentation includes at least 4 vaccinations.
  • TDaP (Tetanus, Diphtheria and Pertussis): Documentation of a TDaP booster within the past 10 years. Renewal date set for 10 years from the administered date of the booster.
  • Polio: Documentation of the completed primary series (at least 3 vaccines) OR positive antibody titer.
  • Influenza: Documentation of a flu shot administered during the current flu season OR submission of the SDSU exemption request based on documented medical or religious reasons. Exemption requests must be submitted annually.  If exemption is approved, additional requirements such as testing and masking may be in place and clinical site placement may be limited.  If placement cannot be made for clinical based on the vaccination requirements of our clinical partners, completion or progression in the program may be delayed or impeded. The renewal date is November 1 of every year.

Professional liability insurance: Documentation of current professional liability insurance coverage is required. Must have limits of $1,000,000 per incident and $5,000,00 aggregate. Renewal date will be set based on the expiration of your coverage.

Additional specialty coverage: e.g. student coverage (NP or CNS) OR APRN coverage. For the the following specializations only: Family Nurse Practitioner, Certified Nurse Specialist, CRNA and Nurse Midwife.

RN license: The student must submit a copy of their current RN license or verification through the state website. The renewal date will be set based on the expiration of your license.

ACLS certification: ACLS documentation from the American heart Association or American Red Cross is required. The renewal date will be set for two years.

These requirements are due prior to your first semester of enrollment (May 1 for summer admission, August 1 for fall admission, December 1 for spring admission). Some documentation will be required to be renewed on an annual basis.

SDSU College of Nursing certificate of health: The student is required to obtain the signature of a licensed health care professional stating that the student is physically able to participate in the nursing program. The certificate of health must be completed using the SDSU College of Nursing certificate of health form and must be signed by a health care professional.

Criminal background check: The student must order a criminal background check through CastleBranch. An annual background check is required.

FBI background check: The student will submit fingerprint to CastleBranch to complete the FBI background check. Fingerprint cards are available through CastleBranch or the Department of Nursing Student Services. Local law enforcement agencies may charge a fee to complete the fingerprints. Results may take 10-15 weeks.

10-panel drug screen: The student will order their drug screen through their CastleBranch account. Drug screen must be completed at a laboratory that is contracted with CastleBranch.

Cardiopulmonary Resuscitation Certification (CPR): Certification must be through an American Heart Association Healthcare Provider or an American Red Cross Professional Rescuers course. Certifications issues in accordance with AHA curriculum are acceptable. The CPR card must be signed. Certificates or letters from provider are accepted temporarily (90 days). The renewal date will be set according to the expiration date of the student’s card.

Tuberculin (TB skin test): Complete documentation for the TB skin test includes the date administered, date read and negative results OR negative Quantiferon blood test and test date OR results of clear chest x-ray. The student’s initial TB skin test must be 2-step; 1-step required for annual renewal. If it has been more than 12 months since the student’s previous TB skin test, a new 2-step will be required. Annual renewal date is based on previous test date.
