Instructions for Scheduling Appointments for Health or Nutrition Clinics from Online Portal

 

  1. Click on Patient Portal button 
  2. Log in with MyState credentials  
  3. At the top of the page, click on Appointments.
    1. Follow the prompts to:
    2. List symptoms/phone number
    3. Choose clinic
    4. Choose appointment reason
  4. Available appointments will then show up.
  5.  Click on the appointment time that will work for you.

If you cannot find an appointment time that fits your schedule, you can call 605-688-4157 to see if we have other available appointment times for that day.
