The Miller Wellness Center believes in providing a healthy lifestyle for all individuals. To make this possible, we are offering all prospective and current members the opportunity to apply for a financial assistance membership. This can help reduce costs for you and your family. What is the Miller Wellness Center Financial Assistance program?

  • At the Miller Wellness Center, we believe everyone deserves the chance to experience a healthy lifestyle. The Financial Assistance program provides families, children and adults with reduced rates based on income so they may obtain a quality wellness membership.

Who is eligible and how is the amount determined?

  • Anyone is welcome to apply for the Financial Assistance program. The amount of assistance granted is based on the application process and review of the required documentation provided by the applicant. Our Financial Assistance program reduces fees but does not eliminate them.

How do I apply?

  • Complete the PDF form

  • Provide all income information and documentation from all individuals in the household.
  • Bring copies of 2 current paystubs and/or other applicable documents from section 2 of the application for each adult in the household.
  • Submit all the information together. Submitting information separately will delay the application process.

  • The Miller Wellness Center will not apply financial assistance to past memberships. Once an application has been approved, assistance can be applied to all relevant memberships moving forward through the expiration date. Please note, you will need to reapply for the Financial Assistance program each year.
  • Financial Assistance will not be applied to programs or services.
