Academic Accommodations

The Office of Disability Services provides assistance for students with a wide range of disabilities. To pursue access to academic accommodations, students must complete the student information form and provide current documentation of disability. These forms, as well as guidance on how to facilitate them, are listed below.

Verification of Disability Form

Some accommodations that can be provided:

  • Accommodations for exams such as time extensions, testing in distraction-reduced environments and/or screen readers.
  • Alternative text formats.
  • Sign language interpreters.
  • Referrals to other resources.

Academic accommodations are determined by an interactive process. The Office of Disability Services and the student will determine reasonable accommodations. Students must request academic accommodations by contacting the Office of Disability Services each semester. Once the student information form and current documentation have been submitted and accommodations have been determined, students do not need to resubmit these forms to the office each semester.

Privacy Statement

The Office of Disability Services respects all privacy and treats all personal information with confidentiality. The office may release information to University faculty and staff when a “need to know” is established by Disability Services. The need to know must be based on compelling and legitimate educational reasons for the information disclosure.
