1. Download the form. The form is a Word document. PLEASE USE THE MOST CURRENT VERSION OF THE FORM (02/08). 2. Complete the computer-based training for researchers. It can be found at http://www.citiprogram.org/. CITI has training focused on biomedical or social and behavioral research. The training takes at least 2 hours, but you can save your progress through the training and continue at a later time. At the end of the training you will obtain a certificate. Send a copy (either a hardcopy or send as an attachment in an e-mail) to Carolyn Curley in the Graduate School Office. All researchers having contact with subjects or working with data that has subject identifiers must complete this training. Certification must be renewed every three years by taking the CITI Refresher Course. 3. Complete the proposal form and submit one electronic copy to SDSU.IRB@sdstate.edu via an e-mail attachment. The proposal will be forwarded to other committee members as needed. Please include your e-mail address somewhere on the form if it is different from the address used to submit the proposal. If your proposal requires full committee review, you must submit at least one week prior to the meeting date. 4. Several signatures are required: the PI, the department head and/or the dean. If the PI is a student (graduate or undergraduate) the student signs as the PI and the student's supervising faculty must also sign. Print the front page of the application, and obtain the appropriate signatures. Send a hard copy of this page to Carolyn Curley in the Graduate School Office. 5. You will be notified by e-mail if any modifications to the proposal are needed or regarding the disposition of your proposal. A proposal that meets the exemption criteria is generally approved in one week. If you are submitting a research proposal that may be reviewed using expedited procedures, your proposal will be forwarded to three committee members for review. The process for receiving approval using expedited review usually requires about two weeks. If you are submitting a proposal that requires full committee review, please submit the proposal no less than one week prior to the scheduled meeting times. 6. You may proceed with your project only when you have received the final approval letter from the committee. |
